In an effort to make the best use of your time and keep your waiting to a minimum, we have our patient forms available to download below. Please download and complete all forms before your appointment.  You may either download the form, fill it out and email it to forms@yadkinvisioncenter.com OR print the form, fill it out and bring it with you to your appointment.

If you are unable to fill out the forms before your visit, please arrive at least 15 minutes prior to your appointment time to allow time for you to do so in our office.

It is also helpful to have the following items on your visit to our office:
– Your current glasses or contact lenses and sunglasses
– A list of ALL medications you are taking
– Your insurance card(s)
– A form of payment for your insurance co-pay and all other applicable charges
– We accept cash, check, Visa/Mastercard/American Express, and Care Credit
